Test Site Registration Form

Registration Procedures:
 

•  ONLINE REGISTRATION
   
• PRINT a Registration Form
• Complete the form, providing all necessary information
• Read  and keep Testing Day Instructions
• Send check or money order with your registration to:
             Homeschool Test Service

  Early Bird Discount:  There is a discount of $5.00 off each test IF postmarked 10 days before the first testing day at your location.

  Registration Deadline:  HTS must receive your testing registration no later than 3 days before the first testing day.
  
  Phone Registrations:  Phone registration is highly discouraged and is being used only in an emergency situation. There will be a $10.00 service charge per child being registered.
  
  Location Change:  Change of test location may be done prior to 2 weeks before testing at the test site for which your children are registered.  There will be a $10.00 service charge for changing locations within that time frame.
